Discusses the four major reasons for construction change orders: unforeseen conditions, customer requested changes, technical errors, and design coordination errors and omissions.  Describes an interdiciplinary coordination design review as one way to detect design errors, which can save the owner, designer, and builder unnecessary, conflict, delay, and unproductive costs.  Reviews architect William T. Nigro's Interdisciplinary Coordination Review System that is recognized by the American Institute of Architecture.
